First of all, I'm so sorry for holding this question over for so long. It was hard to reach the offices I needed to call with their holiday hours.

Are you talking about a missionary deferment or a leave of absence? According to OneStop, "You can take a Leave of Absence for one major semester (fall or winter) per academic year without prior notification to the university as long as you come back to school the next major semester." So if this is a leave of absence and you want to extend it you're going to have to reapply, unfortunately.

If you're talking about a mission, I'm afraid the answer isn't any better. I called and asked OneStop about it and it seems there is no extending your deferment for a mission. However, deferment policy has always been confusing to me and there's a good chance I might have misunderstood her, so please feel free to call and ask with more details. OneStop's number is 801-422-4104.

Of course, there are always extenuating circumstances, and I'd suggest you talk to a OneStop counselor before making any decisions to see if any of them apply to you. The phone number for the counselors is 801-422-7075.
